You can register for the COVID 19 vaccine with your local county health department or in any county that is convenient for you: the state of Texas allows individuals to receive the vaccine at ANY vaccination hub in the state. Large hospital systems and pharmacies and community providers are also registering their communities for the vaccine, so we recommend checking the following resources to find the best way to register for the vaccine for you!
Please note that at this time, all ages 12 and older are eligible for the vaccine. Children ages 0-11 will have to wait to get the vaccine, even if there are appointments available.
You will need your date of birth, contact information and medical conditions. The form is simple. No social security number or government ID is required. All people, of all ages, can register for the vaccine in Dallas. After registering for the vaccine, you will be able to select your appointment time - if eligible for the vaccine.

Dallas Parks and Recreation Department staff at five service centers will be available by phone Tuesday through Thursday from 12 noon to 7 p.m., to assist the public in registering for the Dallas County vaccine waiting list.
